Join the practice
The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the practice Health Care Assistant.
Please note: all child registrations (under 16) MUST have a parent or guardian either registering with them or to have an exising registrations at the practice.
Medical treatment is available from the date of registration. Please contact reception for further information.

Registration for online appointments
You will be given a unique username and password - along with details of how to gain access to the website and the NHS App.
To register for this you need to complete the online registration form and our new patient health questionnaire. We do require proof of ID to register you for this service.
If you experience any problems using this service contact the practice for assistance.
Documents requested for registration
- passport (if applicable)
- asylum seeker card (if applicable)
- proof of name & address e.g. Utility bill (less than 3 months old), council tax bill, driving licence, bank statement
- completed health questionnaire
- children under 16 cannot register without guardian/family
- red book (for all children under 6)
If your circumstances mean that you are unable to provide the above documentation. Please let the practice know when you provide your completed registration forms.
Live within boundary area?
To be eligible to register at the practice, you must live within the practice catchment area. Please see the attached list of roads that lie within our boundary
